MANHATTAN TELEPHONIC NURSE CARE MANAGER- CAN BE LPN OR RN

Client is seeking a dynamic, self motivated individual to participate in the management of care to frail, elderly participants by communicating telephonically with participants, their family members, and medical and nursing personnel caring for them.

Computer skills required.
Ability to work within an interdisciplinary care management team to provide services to frail elderly.
Must be flexible and able to relate well with senior and their families to deal with sensitive issues and facilitate problem solving.
