Job Description:
The primary purpose of this bilingual - Korean/English - position is to provide screening of candidates toward identification of qualified, eligible potentials (related to both nursing home certifiability, Medicaid coverage) for enrollment in the program. This includes but is not limited to the development of referral relationships, case finding and providing potential members and their families with education regarding the MLTC model of care. Involves some traveling in Brooklyn and Queens.Sal based on experience. Amazing benefits!
