Reference Number: Nurse Job Description: Responsible for the planning, coordination, provision, and evaluation of direct nursing service to patients and their families, in the home and coordinators care and services from all other discipline involved in the patients care. Experience Level: Unspecified Requirements: Current New York State License as Registered Nurse One year general Medical - Surgical Nursing experience required Able to meet health standards of employment Must successfully complete orientation Prior home care experience preferred. Must complete four (4) to five (5) visits a day (20- 25) visits per week. After twenty-five (25) visits or more will be FFS payment.
